Fees:

At this time, Monarch Family Therapy, LLC chooses to operate as an ‘out of network’ provider. This is done to focus on the well-being of clients & to avoid dictation of frequency, duration or application from third parties. Receipts are often reimbursable through client's  submission of your monthly receipt(s). 

Payment Methods: Payment is due at time of service. Payment may be made by check, credit card or cash. HSA & Flex Spending Cards are also acceptable forms of payment.

Before your First Appointment:

After speaking with therapist, you will receive an email with an individualized client portal where you can access necessary forms and also pay your bill. For your convenience, forms can be viewed, signed & revisited electronically after providing your email address.

Cancellation Policy:

In order to respect the time of Monarch Family Therapy employees, failure to appear or notify the therapist with at least 24 hours notice for your scheduled therapy appointment, will result in a bill for the full cost of the session.  Exceptions to this policy would include illness or emergencies.
